Medical History form is now online!
LISD has placed the Medical History form online for this year. It is to be done along with your yearly Rankone Online Electronic Forms. All of this needs to be done by May 22nd. Remember that due to the Covid-19 crisis, you will not have to get a new physical for next year unless your answers on the Medical History form indicate a new one is necessary. You will be contacted if you have to get a physical.
